The NSW Department of Health, Chronic Care for Aboriginal People (CCAP) program would like to cordially invite you to the 2011 forum. The theme this year will be:
“Clinical and Cultural Competence in Chronic Disease Management”
This year’s focus will be looking at the four major chronic diseases prevalent in Aboriginal populations across NSW and beyond. The chronic disease categories are:
- Cardiac
- Respiratory
- Renal and
- Diabetes
Chronic diseases affects a disproportionate number of Aboriginal people compared to non Aboriginal people in nearly all age groups and the impact on their daily life and their family is huge.
The aim of this forum will be to explore and identify innovative strategies that are both culturally and clinically appropriate and effective in improving health outcomes in Aboriginal communities. This conference is aimed at Aboriginal and non-Aboriginal staff working together in frontline, management and clinical positions to deliver quality health services to the Aboriginal community.
